When they proactively share those patterns — not just in formal teaching sessions but in low-stakes, peer-to-peer exchanges — junior colleagues absorb the tacit knowledge that formal curricula can't encode.

The Organizational Challenge

The biggest barrier to peer knowledge sharing in hospital settings isn't willingness — most clinicians are eager to learn from each other. It's infrastructure and time. Ward rounds are rushed. Departments are siloed. Night shifts are understaffed. The informal channels that once made teaching hospitals genuinely educational have eroded under the pressure of throughput metrics.

Health systems that have invested in structured peer exchange programs — from Schwartz Rounds to specialty-specific case review panels — consistently report improvements in both safety culture scores and reportable error rates. The common thread is creating protected time and space for clinicians to exchange knowledge without hierarchy or performance evaluation pressure.

Building a Culture of Shared Clinical Intelligence

The shift from a culture of individual expertise to one of collective intelligence is a leadership and systems challenge, not just an individual one. But it starts with individual clinicians choosing to share what they know — their near-misses, their diagnostic surprises, the clinical pearls they picked up from a colleague three years ago.
